Patient Information and Forms
Hudson Retina is a specialty eye practice limited to treating diseases and surgery of the retina, macula, and vitreous. The length of your appointment can vary based on the severity and complexity of your eye condition. Since the practice is referral-based, frequent and unpredictable emergencies are sent to Hudson Retina for immediate attention, and our appointment schedule, at times, may be delayed. The average appointment length is two hours. During your initial visit to Hudson Retina, we will ask you to complete and sign registration and medical history forms. Our office staff will be happy to assist you with this process. Please bring the following to your appointment:
- Current Medical Insurance Card
- Photo Identification
- Names and Addresses of Other Physicians You Are Seeing
- Your Current Eyeglasses
- Any Current Eye Medications
- List of All Current Medications with Dosages
To save time, you may also print and complete the forms and bring them with you to your appointment.
The Process
Once you arrive at our center, our skilled nurses or technicians will check your vision and measure your intraocular pressure, followed by the dilation of your pupils. After a wait to allow your pupils to dilate adequately, additional testing may be performed. Then, you will be examined by one of our doctors. Dilation of the pupils allows examination of the retina and other structures inside the eye. These drops may cause temporary blurring of vision and sensitivity to bright light. We strongly encourage you to bring sunglasses to wear and have a friend or relative drive you home after each visit. New patient visits may require up to 2-hours.
